FOR YOUR INITIAL INTERVIEW PLEASE BRING IN THE FOLLOWING DOCUMENTS THAT APPLIES TO YOUR STATUS. ALL APPLICANTS ARE SUBJECT TO A HOME-VISIT BEFORE THE FINAL MOVE IN IS APPROVED.

 

  1. 1.     A current month to date Social Security Benefit Letter or an official document on letterhead identifying the source of your income; such as General Assistance (GA), EDD-Award Letter, Veteran Income- (VA).

 

  1. 2.   If you are employed full time or part-time please bring in your last four months of paystubs. (Most recent 4 months of paystubs are required).

 

  1. 3.    (A) If you were born in the United States of America or any of its                              

         designated territories a (BIRTH CERTIFICATE IS REQUIRED).

 

       (B) If you were born OUTSIDE the United States of America or any of it

designated territories a (COPY OF YOUR NATURALIZATION

CERTIFICATE IS REQUIRED OR PRESENT YOUR PERMANENT

RESIDENCE CARD ALSO KNOWN AS YOUR GREEN CARD.

 

(C) A valid state identification such as a driver license or a state authorized

senior citizen identification card, retired military identification card or a

valid passport.

 

  1. 4.   A Social Security Card or a Social Security Number Letter from the Social Security Department identifying that numbers has been assigned to you.

 

  1. 5.     A Certificate of Homelessness from any of the designated agencies or a

homeless shelter is required for the Hotel Isabel.
