Case Manager / Outreach Specialist (SOMA / south beach)

Founded in 1974, Swords to Plowshares is a community-based not-for-profit organization that provides counseling and case management, employment and training, housing, and legal assistance to veterans in the San Francisco Bay Area. We promote and protect the rights of veterans through advocacy, public education, and partnerships with local, state, and national entities. We seek qualified applicants for the full-time position of Case Manager / Outreach Specialist. The Case Manager / Outreach Specialist provides assessments, case management, information and referral, and job counseling to assist veterans in becoming reintegrated into the workforce with job placement and/or occupational training programs in accordance with contract goals. Responsible for providing outreach to inform and recruit veterans for employment and training services. Develop recruitment and implementation plan; community outreach and presentation. Develop and maintain database of recruitment sites.

Responsibilities: .

Assess & counsel veterans for job readiness. Responsible for developing; in conjunction with participant, an Employment Development Plan (EDP) to assess barriers and identify individual participant service needs. Determine client eligibility for federal/state employment & training programs . Maintain veteran participant caseload using case management approach. Make referrals to in-house services, as appropriate, including counseling on mental health issues, legal advocacy, job readiness workshops, and pre-employment training . Assist client with pre-employment activities which may include One-Stop Computer Access Point, job search activities . Assist clients in researching training possibilities when needed and enroll in training when appropriate . Communicate with Job Developer on clients’ progress and follow up with clients as necessary . Provide support services, including liaison or linkages with advocacy programs for V.A. benefits and other entitlements; personal counseling services; and community programs for the homeless and/or unemployed. . Maintain concise and up-to-date client files; including required documentation and case/progress notes, complete and accurate completion and execution of paperwork in a timely manner . Ability to articulate and facilitate presentations in a public speaking forum for recruitment purposes . Participate in weekly staff meetings and prepare reports and statistical data as needed for submission . Perform any other employment related tasks as identified and delegated by the Program Manager

Requirements: .

B.A. degree in Social Work, Sociology, Psychology, English, or related degree . Two (2) years experience providing vocational and/or pre-employment counseling and case management in a multi-service setting. Knowledge of federal and state government employment programs a plus. . Excellent written and oral communication skills; excellent organizational and time management skills . Ability to work independently and as part of a team; ability to interact well with people from diverse economic, ethnic and educational backgrounds . Ability to meet multiple deadlines while maintaining a professional composure; proactive; able to prioritize and complete multiple tasks; detail-oriented; reliable, and exercises good judgment; extremely tactful and diplomatic when dealing with people . Ability to operate office computer; word processing experience. . Must be able to sit for prolonged periods, and to perform all functions of this position within what would be deemed to be reasonable accommodations. . Must be able to travel within the Bay area and perform case management / outreach functions at varied sites. Must be able to provide proof of auto insurance
